Financial Aid Application for 2012-2013 School Year

Archway School makes financial aid decisions based on the CAIS Principles of Good Practice and its policy “not to fund students beyond their demonstrated need.” In order to be fair and equitable, the Financial Aid Committee reviews each completed financial aid application. The amount of money available for financial aid awards is limited and is apportioned based on need.

 In order to be considered, the following completed items must be received:

1. A Parents’ Financial Statement (PFS) form filled out and submitted online from www.sss.nais.org. Click on the prompt to begin your Parents’ Financial Statement.

• You will be given a password that will allow you to save and come back to your PFS if needed. Archway School’s code is 3775. There is a small fee to submit the PFS.  The PFS must be completed by January 20, 2012.

In the case of separated or divorced parents, BOTH parents must apply.

1. Please submit a Financial Assistance Inquiry Form (below) to the office by January 20, 2012.

1. Submit IRS Form 4506 http://www.irs.gov/pub/irs-pdf/f4506.pdf to Archway by January 20, 2012. Complete numbers 1-5 and sign but do not submit payment.

1. Upload your 2011 tax return (IRS Form 1040, first two pages, to the SSS website by April 15, 2012.)

To be considered for financial assistance, please adhere to the deadlines. Applicants who apply after the deadlines may not be considered.
